Back in the day, I created a small package of privacy documents and business-related forms for use by the IBCLC private practitioner. So “back-in-the-day” I sold them on CDs that I burned myself.
My forms were and are proprietary … meaning I enjoy and safeguard all copyright protections, which still attach. I am NOT selling the templates any more, because they need to be revised in light of HITECH (now a coupla years old itself), and I do not have the time or inclination to do it. And, no, I am not giving anyone permission to use them, because I reserve the right to get my act together and revise them.
Do not despair. The feds have a free option that is worth checking out. MUCH better stuff started coming from the government during the last Administration than in 2004 when I first drafted my templates. As taxpayers, US-based IBCLCs can see their tax dollars fruitfully at work … and have permission to use and adapt the forms as long as the feds are credited as the source. http://www.hhs.gov/hipaa/for-professionals/privacy/guidance/model-notices-privacy-practices/.
